If you are applying with the Cookeville Fire Department, you will need review and complete the following documentation for additional information.

Applicants should submit their online application through the career portal with or without a resume no later than the deadline noted on the job advertisement/posting. Applicants are required to complete the forms, and bring them on exam day. Do not submit these forms with your application:

Cookeville Fire Department Agility Test with Physician Authorization(PDF)
- Your physician must sign this form
Release and Waiver of Claims Form (PDF)
- This form must be signed by the applicant and notarized
Photograph and Photo Identification (ID)
- Applicants should bring with them a passport style photo of themselves not more than 6 months old.
- Applicant will also be required to present a photo ID (such as Driver’s License) at registration.
