Please give your problem a one line title: For example: password issues, internet issues, printer won't print, toner order, new computer purchase, New User Account, etc

General Help Request

Please supply any details you can, such as the specific software your having a problem with, error messages, the specific issue you need help with, etc. For example: If your trying to print out mailing labels in Microsoft Office Word 2010 and can't figure out how to merge excel values into the document. This allows for general context of the issue rather then saying Microsoft Word isn't working.
This is not required but can slow down the request if not provided. We may need to logon with your credentials to assess the issue when your are not available. If you're going on vacation, to lunch, etc. please provide a password so we may access your computer.

Purchase Request

If requesting small items like toner, ink, external hard drives, etc. please enter item codes in the box above or provide a clear description. For instance: Magenta Toner (HP Magenta Toner MPK-3195), 1TB Western Digital USB Powered HD, two 16GB usb thumb drives, etc. New Computers, Monitors, and other larger items will be discussed during the process to get prices, etc.
If this is a purchase of a computer or another item please include a PO. PO numbers are not necessary for Staples purchases but can be added if you wish.

New User Account Request

Please choose all the options the new user requires. Special access is for areas of the server a user may need to access that is not part of the users department, for example Accounting needing access to HR folders.
Passwords must be 8 characters or longer, must contain 3 out of 4 of the following: lowercase letter, uppercase letter, number, or special character. Passwords can not have been used previously and cannot contain your name.
In cases where a users first name initial and last name match another user, the middle initial will be used to differentiate the username.
Use this area to list departments or folders the user needs to have access to.
If the new user is needing to be added to something besides a windows logon, email, or special account please indicate here. Other type accounts include access to servers, the cookeville website, online software vendors, etc.
